Background

Covers all provider runtime issues: removing FakeListLLM defaults and adding cost controls with fallback.

Expected Behavior

Provider runtime is hardened: FakeListLLM defaults are removed so real providers are used, and cost controls with fallback ensure predictable spending and graceful degradation.
