feat(acms): add ACMS v1 context pipeline
Implement the 10-component pluggable ACMS context assembly pipeline
with three built-in strategies (relevance, recency, tiered), DI-based
component injection, ULID-validated plan_id, largest-remainder budget
allocation, and frozen Pydantic v2 domain models.
Closes #188
