Summary

The [Unreleased] changelog entries for the ACMS context tier hydration fix (#1028) and the sandbox root wiring fix (#4222) are miscategorized. The ACMS item is currently under ### Changed even though it was a bug fix, and the sandbox wiring fix is missing entirely. We need to move the ACMS entry to ### Fixed with clearer wording and add the sandbox wiring fix so the release notes stay accurate.

Acceptance Criteria

The ACMS context tier hydration entry lives under ### Fixed with an updated description that spells out the bug and the behavior change.
A new ### Fixed bullet documents the sandbox root wiring fix (#4222).
All existing ### Added bullets (e.g., Git Worktree Sandbox Apply, Context Hydration Fix) remain present so previously published notes are not lost.
The changelog stays in compliance with CONTRIBUTING.md formatting guidelines.

The original work shipped in PR #4219, but the changelog entry classification was left inconsistent. This cleanup restores accuracy for the upcoming release.
