Epic #935: ACMS Context Assembly Pipeline (v3.4.0)

Problem: Epic had only 3 children (all bug-hunt boundary checks). The 10-component pipeline had no implementation issues.

Fixed: Created 4 implementation issues:

#7400: ACMS pipeline core components (StrategySelector, BudgetAllocator, StrategyExecutor)
#7402: ACMS pipeline post-processing (FragmentDeduplicator, DetailDepthResolver, FragmentScorer, BudgetPacker, FragmentOrderer)
#7403: ACMS pipeline output (PreambleGenerator, SkeletonCompressor, context inheritance)
#7398: CRP tool handlers (request-context, query-history, get-context-budget)

Epic #934: LSP Runtime Implementation (v3.5.0)

Problem: Epic had only 5 children (all TEST-INFRA issues). The LspRuntime was entirely a stub with no implementation issues.
